Description The Community Liaison RN is responsible for creating
and maintaining positive customer relationships to increase
community awareness and referrals for Home Health, Hospice, Home
Infusion, PACE, Transitional Care Unit and other services within
assigned accounts. This role includes working with discharge
planners, facility staff, physicians, and individuals to assess
appropriateness for hospice and home health services and facilitate
coordination of services, and to ensure appropriate documentation
for compliance with agency and statutory guidelines to manage the
transition of the patient from an acute care facility to the
patient's home or next location of residence. Providence caregivers

them. Required Qualifications:
- Education sufficient for licensure
- California Registered Nurse License upon hire.
- 1 year Full-time experience in clinical role. Preferred
Qualifications:
- Bachelor's Degree in Nursing
- Liaison, marketing or sales experience.
- Health care industry experience.
